Free Customized Photo Books to any family with a relative struggling with memory loss disorders. 

To get a Photo Book, you must fill out an application below asking some background date

Customers will be required to fill out a google form asking for a memory book desired page number, and will be required to send in 30 or more photos of their loved ones to be turned into a memory book. They will also be required to send in some survey data for example there will be demographic information about who the book is meant for, what kind of neurodegenerative disease do they have, and then they will sign a consent form stating they will answer some progress questions 1 month, 3 months, 6 months, and 1 year after they receive their memory book. This data will be used to analyze if memory books are more advantageous for certain types of neurodegenerative diseases, and to see how long it took to help their loved ones, and general feedback to improve service. After the google form is completed their photos will be sent to a volunteer to create their memory book. This process will take approximately 2 weeks to complete after the order has been processed. After 2 weeks, the customers will receive their e-book through their email or physical book in the mail.
